Declaration of Consent for processing Personal Data – Contact Form

1. I give my explicit and informed consent to ” SKY Asset Management” AD, a company duly registered in the Commercial Register of Bulgaria with UIC 175040675, with address in Sofia, Oborishte district, 31 Ekzarh Yosif Str. (“the Company”), in its capacity as personal data administrator, to process the data specified below for the following purposes, namely :

Enabling clients and potential clients to receive answers to specific inquiries submitted through the contact form on the website www.skyfunds.bg .

I hereby also declare that:

• I am aware that the consent I have given to process my personal data is valid and constitutes a valid legal basis for processing my personal data until its explicit withdrawal by me, and I can exercise this right by sending an application to the Company at the above address or to the following contact e-mail address – contact@skyfunds.bg .
• I am familiar with the information under Article 13 of the General Data Protection Regulation, as well as with the following rights: (i) right of access to my personal data; (ii) the right to request correction, modification and updating of this data, (iii) the right to request restriction of their processing, (iv) the right to request the Company to delete (erase) or block my personal data, the processing of which does not meet the requirements of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation), as well as to notify third parties to whom these data have been disclosed of any such deletion or blocking, except where this is impossible or involves disproportionate effort, as well as (v) the right to object to further processing of my personal data where there is a legal basis for this.
• I am aware that the provision of personal data for the above purposes is not mandatory, nor a contractual requirement, or a requirement necessary for the conclusion of a contract, and that I am not obliged to provide personal data for the above purposes, and that there would be no consequences for failure to provide it .
